Catalan family doctors propose the ‘autobaja’

Catalan family doctors, grouped in the Catalan Society of Family and Community Medicine (Camfic), have proposed that the absences from work due to covid-19 they can self-justify in cases in which the workers have had an antigen test at home and have communicated the result through the pharmacies.

Camfic also advocates extending the period for which discharge can be extended from the first day of sick leave after seven days in case of covid.

“This would not prevent that, if after this period, the patient remains symptomatic, you can contact the health center to prolong temporary disability“, indicates Camfic.

This model, justify the family and community doctors, “is being applied de facto” in Catalonia by registering the antigen tests and considering truthful telephone information received, which allows minimizing the displacement of those infected.

They have also recalled that in other countries with similar health organizational models, such as the United Kingdom, there are so-called ‘responsible self-declaration forms‘ for any contingency for a period of seven days, already existing before the pandemic, that during this situation have been extended to contingencies of up to 28 days.

Camfic today urged the competent bodies to “establish the regulatory changes nnecessary to implement this formula of self-responsible declaration, which encourages the mature attitude that society demands and that will contribute to reducing one of the main reasons for ‘burnout’ (exhaustion) of the professional group but, above all, to reduce excess bureaucracy without clinical value for the person being cared for”.

Other pathologies

The sixth wave of the covid pandemic is causing a “historic influx” in the number of visits assisted in the CAP (more than 100,000 views from covid alone on the 10th), a fact “that compromises the quality of care, patient safety, and contributes to the demostrado infradiagnóstico of other non-covid pathologies with a high impact on morbidity and mortality”, the family doctors have assured.

According to a study prepared, among other primary care experts, by Dr. Antoni Sisó-Almirall, president of Camfic, the impact of covid-19 on new cases of diseases and common pathologies in primary care in 2020 meant significant reductions in hypertension (reduction of 40%), hypercholesterolemia (36%), type 2 diabetes (39%), chronic kidney disease (43%), ischemic heart disease (48%), osteoporosis (40%) and chronic obstructive pulmonary disease (50%), among others.

This study states that “the prioritization of covid-19 care changed the doctor-patient relationship to the detriment of scheduled face-to-face visits for the detection and monitoring of chronic diseases, which were reduced by almost 41%”.
